Healthcare Case Manager
Job Summary
The Healthcare Case Manager is responsible for coordinating patient care, assessing healthcare needs, and ensuring that patients receive appropriate medical and support services. This role involves working closely with medical providers, patients, and families to create individualized care plans, monitor progress, and facilitate access to necessary resources. Ideal candidates have strong clinical knowledge, communication skills, and a commitment to improving patient outcomes.
Responsibilities
- Assess patients' healthcare needs and develop comprehensive, individualized care plans in collaboration with healthcare providers.
- Coordinate patient services, including medical appointments, home healthcare, rehabilitation, and support services.
- Monitor patient progress and adjust care plans as necessary to meet changing health needs.
- Provide patients and their families with education, support, and guidance on managing health conditions and navigating the healthcare system.
- Advocate for patients to ensure they have access to needed resources, services, and benefits.
- Maintain accurate, up-to-date patient records, adhering to confidentiality and compliance regulations.
- Collaborate with a multidisciplinary healthcare team, including doctors, nurses, and social workers, to provide integrated patient care.

Preferred Skills
- Previous experience in case management or a clinical healthcare setting.
- RN, LPN, or LCSW licensure is often preferred for healthcare case managers.
- Familiarity with case management software and electronic health records (EHR).
